Dr. Silverberg is conducting a survey to review medical, genetic, psychological and nutritional factors that may cause or exacerbate vitiligo.

Individuals with vitiligo that have been previously diagnosed by a physician  are welcome to contribute to this survey.

The survey will not include any personal information that may identify you to the public.

VITILIGO, THE PATIENT'S POINT OF VIEW

Wherever you live, at Loyola University Chicago we want to hear about your case of vitiligo. This helps us direct our research, and to find support for our current thoughts on progressive depigmentation. The questionnaire should take between 5 and 10 minutes to complete. Please complete the attached form and send to the email address below to have the results included in our database. We intend to publish the data once ample questionnaires have been returned to us.
